You are offered more health care options than ever before. Keeping the costs of that care under control is not only up to employers and providers - it's up to you.

In 2013, Banner is offering you three pharmacy benefit choices - Basic, Standard and Expanded.


Basic Plan: The Basic Plan is best suited for those who:

  1. Currently take no prescription medications regularly. ľOR-

  2. Are taking some medications, all of which are listed on the Basic/High Performance Formulary.

  3. Request generics instead of brand-name medications whenever possible.


Standard Plan: The Standard Plan is best suited for those who:

  1. Regularly take medications that are not listed on the Basic/High Performance formulary.

  2. Have all/most of their medications listed on the Prime Formulary.

  3. Take some brand-name/non-formulary medications regularly.


Expanded Plan: The Expanded Plan is best suited for those who:

  1. Regularly take medications that are not listed on the Basic/High Performance formulary.

  2. Have all/most of their medications listed on the Standard-Expanded/National Preferred Formulary.

  3. Take several brand-name/non-formulary medications regularly.


For 2013 open enrollment, the Express Preview web site has been set up to assist you in selecting one of the three pharmacy plans. A fourth informational-only plan has been set up to demonstrate the pharmacy cost you would incur if you had no insurance coverage.

Using this website, you can compare each plan for an ESTIMATE of specific medication costs and your total annual out-of-pocket costs to decide which plan best meets your needs.
